What Services Are Offered by Assisted Home Care?

Assisted Home Care provides the short-term intermittent nursing or therapy visits prescribed by your physician. Care can be arranged for infants through adults.

Care is typically initiated following a hospitalization (or short-term stay in a skilled nursing facility). With hospital stays shorter and shorter, home health helps transition you back to health in the comfort and security of your own home.

Your physician can also prescribe home health care if there is an recent change in your health status or an exacerbation of an existing condition.

To qualify for home health visits you must be homebound – meaning it takes a taxing effort to leave the home. Care must be restorative or rehabilitative, not custodial.

Intermittent visits involve a nurse or therapist coming to your home to perform a clinical procedure or to guide you through a therapy session. The care must be medically necessary and involve the services of a licensed professional. Services include:

Registered Nurse
Physical Therapist
Occupational Therapist
Speech/Language Pathologist
Medical Social Worker
Home Health Aide (in conjunction with licensed care)

Who Initiates Service?

Your physician prescribes the care. If you feel you qualify for this service, contact us and we will partner with your physician to determine if you qualify for intermittent home health care.

What Can I Expect Once My Doctor Orders Home Healthcare?

  • A nurse or physical therapist will call you to schedule your first visit. If you have been hospitalized, the first visit is usually the day after discharge, unless otherwise medically indicated.
  • Following the evaluation, the nurse or therapist and your doctor will develop a plan of care which includes the frequency of visits. Your input and participation are an important part of developing this plan.
  • Visits are scheduled allowing for some flexibility regarding the time you can expect us to arrive at your home. Time flexibility allows us to provide each patient with individualized care.

How Are These Services Reimbursed?

Intermittent home health care is covered 100% by Medicare. Most private insurance policies cover home health care services as well. Assisted has contracts with most carriers. We will pre-verify your home health benefits to ensure there are no surprises.
